Firearm Transfer Information
Thank you for choosing Utah FAST for your firearm transfer!
Completing this form helps us ensure your transfer is completed quickly and reduces errors in our system. This form does not replace or autopopulate the Firearm Transaction Record (ATF Form 4473).
The transferee/buyer must be the individual who purchased the firearm as shown on the order/invoice, or we will not complete the transfer.
IMPORTANT!
You must provide acceptable identification that includes your current residential address. This includes a valid, non-expired government-issued photo identification, such as a driver’s license or state-issued identification card, including a temporary (paper) driver’s license, that displays the buyer’s full legal name, date of birth, and current residential address.
Identification that is not accepted includes any ID with a handwritten sticker indicating an address change, any expired ID, or a driver’s privilege card. If the ID does not have the buyer’s current residential address, supplemental identification can be used.

Shipping Dealer Information
You must complete the following shipping dealer information accurately. This automatically sends an email to the selling dealer with your full name, order number, a copy of our FFL, and, if applicable, our SOT for an NFA firearm. If this information is incomplete or incorrect it will delay our receiving your firearm.
